Just some general advice - you don't have to combine ALL of your interests into a career. For example, you could be a registered dietician and on the side foster dogs rescued from shelters and participate in your town's environmental commission.
Or be a vet tech (I am not suggesting veterinarian because it is a very competetive field and you would need excellent math grades to get into vet school) and volunteer to help people on the side while being eco-freindly in your personal life.
Try some volunteer opportunities if you haven't already in your fields of interest. You might realize that as much as you like helping people or animals, doing it every day as a job might burn you out OR, hopefully, you would realize that you have a true passion for one more than another.
Try to get extra math help or even try online math videos. You might be better at it than you think.
